Privacy Policy 

 

Our privacy notice tells you about the information we collect from you when you use our website and our service. By law we are required to inform you about who we are, about how and why we use your data and about the rights you have over your data. As a sole trader business, we have registered with the Information Commissioner’s Officer in line with UK regulations.  

 

Who we are

 

Quality CaRE Oncology Physiotherapy (“us”, “we” or “our”) is the trading name of Carla Reid (Sole Trader). We are a private physiotherapy service providing physiotherapy to the public virtually, within their homes or community. We can be contacted via our email at: info@oncologyphysio.com

 

What information we collect

 

Whilst using our service we collect “Personal Data”. This is data that contains personal identifiable information that can be used to contact or identify you.  

 

When you book an appointment via our website by email, we will ask for your name, email address and telephone number. 

We will contact you vitually or by telephone to gain verbal consent to collect your address, only if doing home visits, and your date of birth. We will then take a relevant brief medical history and a brief history of your presenting condition. We will confirm your booking by telephone or email followed by a delivery of our service virtually or within your home based on your booked appointment.

 

When we provide our service, we will obtain your consent in line with the Health and Care Professions Council Principles of Consent (2025)

If you are under the age of 16 years old, we will obtain verbal and written consent from your parent or legal guardian.

 

In accordance with the Quality Assurance Standards of Physiotherapy Service Delivery (2017) we will collect data in the form of clinical documentation. This includes your contact details, relevant medical history, consultant, general practitioner’s name and practice, medical insurer’s details, and the therapy assessment and intervention in the form of SOAP notes (Subjective history, Objective history, Analysis of intervention and Treatment Plan).

 

We aim to keep your medical records secure if electronically stored, password protected, or within a locked file, if in paper format.

 

Medical records will be kept for 8 years following the last appointment entry and 3 years following death. If you are under 18 years old, notes will be kept until you 25th birthday. Following this your records will be destroyed securely and confidentially in accordance with the Chartered Society of Physiotherapy (2025). 

Were necessary, in your best interest and with your consent, data may be shared with your general practitioner and other health care professionals in the form of reports, recommendations and ongoing referrals to support your care. You will receive a copy of any such written communication.

 

You have a right, by law to ask us about the information we have about you and to correct it if inaccurate. This is commonly known as a data subject access request.

 

In line with the principles of the Data Protection Act (2018) the data collected by Quality CaRE oncology physiotherapist will ensure that your personal data is:

 

  • Processed fairly and lawfully.

  • Used for specified, explicit purposes.

  • Adequate, relevant and limited to only what is necessary.

  • Accurate and, where necessary kept up to date.

  • Kept for no longer than is necessary.

  • Processed in a manner that ensures appropriate security.

 

Your Rights

 

  1. You have the right to be informed about the use and collection of your personal data. At Oncology CaRE Physiotherapy, we aim to be transparent with you about your data.

  2. You have a right to access the personal information we hold about you. To request this information please make a subject access request in the form of writing.

  3. You have the right to rectify any information we hold that may be incorrect.

We strongly advise you to review the Privacy Policy of every site you visit. We have no control over third party sites and assume no responsibility for their content, privacy policies or practices.  

Withdrawal of Consent

 

Where you have provided your specific consent to the use of personal information, you may withdraw that consent by contacting us.

Complaints 

 

If you have a complaint about the use of your information we would like you to contact us directly in the first instance for us to try and address your complaint. However, if you are dissatisfied with how we deal with your complaint you can also contact the Information Commissioner’s Officer via their website at https://ico.org.uk

 

Changes to This Privacy Notice

 

We may update our Privacy Policy from time to time. You are advised to review the Privacy Policy periodically for any changes. Changes to the Privacy Policy are effective when posted on this page.
